Required Adoption Documents

Please make copies of all the documentation and have a notary or lawyer notarize the documents as true copies of an original document. The notary or lawyer needs to place a gold or red seal on the document, impress his seal, sign, date, and include his address. All documents are valid for one year except where noted.

Russia now requires TWO (2) complete sets of these documents.

1. Passports

Picture page of your passport. (3 copies for Kazakhstan)

2. Marriage Certificate.
If you are a single person adopting you will need to include a birth certificate. (3 copies for Kazakhstan)

3. Criminal Record Clearance
Obtain either the Original which can be copied and notarized or two Certified True Copies. You will do this for your agency. Once they receive it, they will forward it with the other documents they complete. See below.

4. Letter from Employers (verifying employment and income).

Ask your employer(s) to prepare a statement on company letterhead which covers the following information: length of employment, current position and last calendar year's salary.

5. Medical Report (Form enclosed) - DO THIS AS ONE OF YOUR LAST STEPS.
Ask your medical doctor(s) to complete and sign the medical form titled Letter of Approval. Please also have him stamp the document with his name and address as sometimes their signature is hard to read. Valid for three months.

6. Personal Statement of Assets and Liabilities

Ask your Financial Institution to prepare a letter on their letterhead stating how long you have been a customer in good standing, how much you have on deposit and outlining your main assets and liabilities. A similar letter may be prepared by an accountant.

7. Letter from Agency stating who you are, when your home study was completed, summarizing who you have been recommended to adopt. This document along with the Agency license must be attached to your home study and must state the number of pages in your home study.

8. Approved International Home Study (completed by a licensed agency). Each page of the home study should be numbered and each page must bear the stamp of the notary.

11. Application for Adoption (Form enclosed)

12. Statement of Obligation (Forms enclosed) from the parents stating that they will register the child at the Russian or Kazakh Embassy on returning to Canada.

13. Photographs

Please include one color photograph of the following: a close up of yourselves, a close up of family members, the outside of your house, apartment or condo front and back, the park and schools in your area, your neighborhood, and a picture of the rooms in your house. Be sure to label each photograph.
